You may need a workers compensation insurance policy, employer account numbers, and Utah unemployment insurance registration number.
It is a certificate of insurance showing your contractor general liability policy. The insured business name, coverage limits, dates, and certificate holder should match the current DOPL instructions.
DOPL general liability certificateBusinesses with employees may need workers compensation coverage. Applicants with no employees may qualify for a Utah Labor Commission waiver. Worker classification should be reviewed when the answer is unclear.
Utah workers comp waiver for contractorsIf your business has W-2 employees, Utah may require a withholding tax account with the Utah State Tax Commission. This is the account used to report and remit Utah income tax withheld from employee wages. It is separate from workers compensation and unemployment insurance.
If your business has W-2 employees, Utah may require an unemployment insurance registration number through Workforce Services. This is used for unemployment tax/reporting if employees later lose work and qualify for unemployment benefits. It is not a workers comp policy and it is not money the business controls.
The business name on your insurance certificate should match the legal business name on your DOPL application. If you are using an LLC, corporation, sole proprietor name, or DBA, the insurance setup should be reviewed before the COI is issued.
That is normal if you have not set up the GL policy yet. The DOPL COI comes after the policy is bound. If you do not have a COI, the next step is usually figuring out the right GL policy for your trade, business name, employee status, and target submission date.
